Mechanism of
action
Thiamin (vitamin B1) combines with adenosine
triphosphate (ATP) to form a coenzyme, thiamine pyrophosphate (thiamine
diphosphate, cocarboxylase), which is necessary for carbohydrate
metabolism. Riboflavin (vitamin B2) is converted to 2
coenzymes, flavin mononucleotide (FMN) and flavin adenine dinucleotide
(FAD), which are necessary for normal tissue respiration. Riboflavin is
also required for activation of pyridoxine, conversion of tryptophan to
niacin, and may be involved in maintaning erythrocyte
integrity. pyridoxine (vitamin B6) is converted in
erythrocytes to pyridoxal phosphate and to a lesser extent pyridoxamine
phosphate, which act as coenzymes for various metabolic functions
affecting protein, carbohydrate, and lipid utilization. pyridoxine is
involved in conversion of tryptophan to niacin or serotonin, breakdown of
glycogen to glucose - 1 - phosphate, conversion of oxalate to glycine,
synthesis of gamma aminobutyric acid (GABA) within the CNS, and synthesis
of heme. Nicotinamide, is a component of two coenzymes, nicotinamide
adenine dinucleotide (NAD) and nicotinamide adenine dinucleotide phosphate
(NADP), which are necessary for tissue respiration; glycogenolysis; and
lipid, amino acid, protein, and purine metabolism. Dexpanthenol is the
alcohol analog of D - pantothenic acid. Pantothenic acid is a precursor of
coenzyme A, which is cofactor for enzyme - catalyzed reactions involving
transfer of acetyl groups.
Indications
Vitamin B deficiency (prophylaxis and treatment). This
vitamin supplement is also indicated in: Elderly patients to improve
their disease; concurrent with broad spectrum antibiotics after surgery.
Precautions to
considerHypersensitivity to
any of the vitamins content in the preparation.
Pregnancy / Breast -
feeding
Problems in humans have not been documented with intake
of normal doses.
Drug
interactions
Combinations containing any of the following medications,
depending on the amount present, may also interact with this
medication. Vitamin B2 : Alcohol; Antidepressant, tricyclic;
Phenothiazines; Probenecid. Vitamin B6 : Levodopa;
Phenobarbital; Phenytoin; Isoniazid;Hydralazine;
Immunosuppressants. Nicotinamide: Lovastatin. Dexpanthenol:
Antibiotics; Barbiturates; Narcotics.
Side / Adverse
effects
- Allergic reaction, anaphylactic (skin rash or itching; wheezing).
- Large doses may cause yellow discoloration of urine.
Administration and
dosage
Prophylaxis: Adult: 1 - 2 teaspoonfuls
per day. Children: 1/2 - 1 teaspoonfuls per
day. Treatment: Treatment dose is individualized by
prescriber based on severity of deficiency.
